Far lateral disc herniations are less common than the ususal posterior or posterolateral disc herniations and affect the exiting rather than the traversing nerve root. For example a far lateral L3-4 disc herniation (foraminal or extra-foraminal) compresses the L3 nerve root, whereas a posterolateral disc herniation affects the L4 nerve root. The surgical technique involves a more lateral exposure to unroof the neural foramen and decompress the exiting nerve root.
