The clinical presentation, radiographs, and pathology are consistent with osteosarcoma. The radiographs show a mixed lytic/blastic destructive lesion. The pathology specimen shows pleomorphic osteoblastic cells and lacey osteoid. Therefore, the answer to this question is neo-adjuvant chemo and wide resection.

In the referenced study by Bacci et al they reported that the “The five-year disease-free survival and overall survival rates were 60.7% (limb salvage) and 68.5% (amputation), respectively, with no significant difference between patients undergoing amputation and those undergoing resection.”
