Dr. Pakzaban (www.pakzaban.com) demonstrates a lateral foraminotomy and microsurgical resection of a far lateral disc herniation. Far lateral disc herniations are less common than the ususal posterior or posterolateral disc herniations and affect the exiting rather than the traversing nerve root. For example a far lateral L3-4 disc herniation (foraminal or extra-foraminal) compresses the L3 nerve root, whereas a posterolateral disc herniation affects the L4 nerve root. The surgical technique involves a more lateral exposure to unroof the neural foramen and decompress the exiting nerve root.
