Monteggia Nonunion in 45M

HPI

A 45-year-old male fell from a height one year prior and underwent open reduction and internal fixation (ORIF) of the ulna and a lateral ligament repair at an outside institution. He now presents with continued right elbow pain, deformity, and loss of function. The patient reports minimal and painful pronation and supination of the forearm.

PMH

Unremarkable. He is a non-smoker.

PE

On inspection, the incision is well healed without signs of erythema or cellulitis. There is an obvious deformity of the arm. His range of motion is limited and he has mild pain with pronation and supination of the forearm. His radial pulse is palpable. Sensation is intact to the radial, ulnar, and median nerves in the right hand.
