Hip Arthritis in Obese 56M with BMI of 53M

HPI

A 56-year-old male presents with longstanding bilateral hip pain and osteoarthritis. His pain is significantly worse on the right. He has had prior bilateral knee replacements. He works as an electrician which is significantly impacted by his hip pain. The pain has progressed to the point that he can not walk significant distances.

PMH

He is morbidly obese. Otherwise no significant medical issues. He smokes ½ pack of cigarettes daily.

PE

On physical exam, he is 5'7", 340 lbs, and has a BMI of 53.3. The skin is clean over the anterior hip with no rashes or skin breakdown. He has painful ROM of bilateral hips with no contractures, worse on the right.
