Glenohumeral Arthritis in 58M

HPI

A 58 year-old-male presents with progressively worsening shoulder pain over the last 10 years. His pain is severe enough that it limits much of his overhead activities. He has difficulty sleeping at night due to shoulder pain. He completed physical therapy and had 3 injections over past 5 years, none of which provided lasting relief of his symptoms.

PMH

He is otherwise healthy and active with mild hypertension.

PE

On physical exam he has 130 degrees of forward flexion, 70 degrees of external rotation, and 5 degrees of internal rotation. He has 5/5 strength in all planes for rotator cuff strength.
