A 65-year-old woman presents to an orthopedic adult reconstruction clinic with a 3-month history of worsening left hip pain and lower extremity weakness despite activity modification and oral NSAIDs. She reports left hip and lower back pain that has bothered her for several years; however, over the past 3 months it has become more difficult to walk. In the clinic, she walks with a wide-based gait. She has pain in her mid and lower back with palpation and has pain with flexion and internal rotation of the hip that is focused in the groin. She is hyperreflexic in her bilateral lower extremity reflexes with notable clonus. She denies any change in her upper extremity findings and has a benign cervical spine exam.
