A 34-year-old morbidly obese male presents to your office one week post-operatively following a massive rotator cuff repair. The case took longer than usual for you due to the complexity. During discussion with the patient, he notes that his shoulder pain continues to improve each day and is well controlled on the pain medication. However, he complains of significant pain and burning down his outer right thigh. On physical examination, you find that this does not go below his knee and there is no evidence of muscle weakness on testing. You do recall not being present for his set-up as you were talking with the previous patient's family. Based on the patient's complaints and your physical examination, appropriate positioning and padding over what area(s) on Figure A would have helped prevent the patient's current complaints?
