A 37-year-old male presents to your office with complaints of medial right knee pain of multiple years duration. He is a construction worker and states he is now having trouble performing his daily duties. He has done physical therapy and been given a steroid injection by a different provider with little relief. On examination, his knee demonstrates varus alignment and tenderness is present primarily over the medial joint line. Radiographs are obtained which demonstrate a varus deformity of 8º of the right knee with moderate arthritis in the medial compartment only. You offer him a medial unloader brace which does passively correct his deformity and improve his pain but he is unable to tolerate it while he is working.
