A 39-year-old male laborer has long-standing lateral elbow pain that is worse with gripping activities and has noted some decreased grip strength. On exam, the pain is exacerbated with resisted wrist extension and maximal wrist flexion. He fails conservative treatment and undergoes arthroscopic surgery. Postoperatively he is recovering well and his pain resolved but when he resumes exercising he complains of lateral elbow pain with clicking and catching and is unable to do pushups. Figure A is a view from the proximal anteromedial portal. Excessive debridement beyond which point may explain his symptoms?
