This is a tough injury in a younger male laborer, the ER lag sign and weakness is a significant concern in addition to the degree of tear/retraction! Ideal to maintain native joint here, if possible repair/augment the cuff and potential tendon transfer as has been suggested!

With lag signs, preserved overhead motion, & massive retracted RCT in an active male w/ manually intensive job, I would lean heavily towards a tendon transfer w/ lower trapezius. Attempt a repair to residual tendon and biceps centralization (if still intact). Good case!

agree that this needs a motor-impacting solution because of the loss of power as evidenced by the lag signs which for him at 54 pushes you more towards a LTT, same issue at 70 a rTSA with lateralization (dealers choice on how you get there) would be a good choice, however an old-school grammont styl...e medialized device would likely require concomitant tendon transfer as well. nice case, Raf!

Is this click bait!? MRCT, not fixable. Even if you fix it, it wont heal. This is dealers choice in my opinion. I think this would do well with a Speed Trap ( lower trap transfer) , Bio Tuberoplasty, or RSA . I think with this range of motion and his young age combined with high activity level, my p...referred surgery would be a Lower Trap transfer. This gets you a biologic humeral head interpositional arthroplasty effect, it gives you humeral head depression effect, and it can increase strength in ER but also in scaption a little. He would have no restricitons with lifting once healed. Although he needs to buy into the Gunslinger for 6 weeks and the long recovery. These are the biggest drawbacks.

The patient would be informed that a sling is not there to make the repair heal - the sling is to stop him doing silly things while the repair heals!
