Extensile Exposure for a Radical Limb Sparing Resection of an Axillary Sarcoma

Video Description

This video demonstrates the reconstruction of a sarcoma arising from the axilla. The axilla is a rare site to develop a sarcoma. Tumors in this location pose a surgical risk because they are close to the major neurovascular structures of the upper extremity and are typically large at presentation. They can grow proximally into the neck and thoracic outlet, and distally into the arm and chest wall. The extensile exposure described in this video permits full visualization of the axillary vessels and brachial plexus. These structures can be safely dissected and mobilized away from the tumor, enabling limb sparing resection. This is a limb sparing surgery as opposed to performing a forequarter amputation of the arm. Specific protocols for physical therapy and use of a sling or a brace for a period of time must be adhered to postoperatively for an optimal result.
