With the patient in the prone position, place the base of one hand over the apex of the sacrum, reinforce it with the other hand, and then gradually apply a vertical load directly towards the table. The loading is intended to create an anterior shear of the sacrum relative to the innominate. This test is considered positive if the patient complains of pain in the region of either sacroiliac joint.
